The tool bar rarely displays on an iPad

This is unique to iPad OS. This worked as normal until a few months ago.

Then the tool bar (indent, "+" "@" bullets etc) disappeared. On rare occasions it will display for a few seconds, then go away. Sometimes it will the top half of the line; ie its cut off. But, most of the time there's a grey bar where the icons used to me.

Note: I have found this is unique to iPads.
